Hardship Waiver

As the qualifying relative, spouse or parent, of the beneficiary (son/daughter or husband/wife), you are required to demonstrate to the satisfaction of USCIS that you will suffer extreme hardship if your son/daughter or husband/wife is denied the green card and is deported or excluded from the United States. To assist us, your attorneys, to represent and present your waiver and to increase the chance of approval, We, and USCIS, need to understand you, your relationship and your family life. Thus, it is important that you take some time and write an detail Statement about your life, health, relationship, and events in your life. I need you to answer the following questions and provide supporting documents:

List all brothers and sisters in the United States

List all medical issues you, your children and spouse have had, and provide a physician letter or emergency room record substantiating the medical issue?
Child's Schools / Activities

Name and address of the schools your children currently attends and provide school records and certificates/honors that your children has received. If there are in any special classes or programs/sports please list?
